APPLICATION CHECKLIST

If you are invited to submit a full application, you will need to submit several items; please review this checklist to ensure you can supply all of the necessary documentation:

• Completed and signed application and forms (both applicants if applying as a couple)
• Letter of application stating your need for a Habitat home and willingness to partner with us
• Authorization to check credit records, employment, bank balances, and Social Security Income
• Copies of federal tax returns from the past two years (please do not send originals)
• Copies of W-2's from the past two years (please do not send originals)
• Copies of employment pay stubs from the past two months
• Copies of bank statements from the past two months
• Explanation of any bankruptcy, judgment, or other financial problem

Please make a copy of your completed application for your records.
